Governor Lamont has authorized that State of Connecticut hair salons may
reopen on June 1, 2020
under Phase I of his Road Map to Reopening Connecticut from the Coronavirus
(CV-19) pandemic.
You acknowledge that you have chosen to re-enter Escape Salon and have
hair salon services and
products provided to you by Escape Salon employees.
Escape Salon has reviewed and performed the preventive actions set forth
by the Connecticut
Department of Economic and Community Development Reopen Connecticut Program
and will
adhere to the Connecticut Rule under that program and has Self-Certified
its adherence as required
by law. Although Escape Salon has taken all required safety steps as required
under Connecticut
law, you acknowledge that under the COVID virus (CV-19) pandemic that
it is impossible to ensure
that Escape Salon can protect you from contracting the COVID virus (CV-19).
You agree that your signature below indicates your consent to the terms
of this waiver and
your agreement to indemnify, defend and hold harmless Escape Salon, its
officers, directors,
shareholders, employees, independent contractors, and agents from and
against any current
or future, liabilities, claim or claims, lawsuits, costs (including reasonable
attorneys’ fees and
costs), or actions of any kind, whatsoever for liability and/or damages
arising from entering
and having the salon services and products you have chosen to receive
from Escape Salon.
